If you save the PowerPoint to a service like SlideShare (or view it in Google Docs Viewer) you can present it in a custom webpage that has e.g. a reduced size, embedded slide presentation on the left and an embedded Twitter stream on the right. Blogs make use of this all the time with e.g. SlideShare presentation in a post and a Twitter widget in a sidebar. It's a kludge..but it'll work. But if you're going to go down this path you might as well just show a Google Doc presentation in a browser stretched to 4/5 of the your screen's width and your Twitter app taking up the remaining 20%. But then you already knew this :^) Alas, I can't think of a more elegant solution.

Assume you're a Windows user so you might find one of these "stay on top" window utilities works (pin your twitter client as topmost window) then run your powerpoint slideshow.

Afloat will even make a translucent skin so you can have the twitter stream running full-screen on top of your presentation--although that might get kinda confusing to your audience.
