I want to make a form like those created by survey monkey/google forms, but that allows the user to upload files (PDF, mostly). Free is a must, can be web based or self hosted, though.

I don't think contact form 7 allows uploads, does it? I had not looked at wordpress plugins, though. hmmmm....
A little more context: I'm on a committee that will be awarding a scholarship, and we are looking for a way to collect the applications - so in addition to filling out a form, the user will have to upload a resume and potentially other files. Software that handles employment applications would probably work, but I can't find a free solution for that.

The documentation says it does! Full disclosure -- I've never worked with any WP form plugins. But given what you're describing and what I'm seeing in the CF7 docs, it's probably worth a try.
